Overview

Vitamin D is an essential vitamin that helps regulate calcium and phosphorus in the body. It also plays a role in maintaining proper bone structure.

There are different forms of vitamin D, including ergocalciferol (vitamin D2) and cholecalciferol (vitamin D3). Vitamin D is found in fish, eggs, and fortified milk. It's also made in the skin when exposed to sunlight. During periods of sunlight, vitamin D is stored in fat and then released when sunlight is not available.

Vitamin D supplements are commonly used to treat and prevent vitamin D deficiency. People who don't get enough sun and people who are 65 years or older are at risk for deficiency. People also use vitamin D for weak and brittle bones, heart disease, asthma, hay fever, and many other conditions, but there's no good scientific evidence to support many of these uses. There is also no strong evidence to support using vitamin D supplements for COVID-19. But it is important to maintain healthy levels of vitamin D. This can be done by taking 400-1000 IU of vitamin D daily or spending 15-30 minutes in the sun each day.

Uses & Effectiveness

Effective for

A rare, inherited bone disorder marked by low levels of phosphate in the blood (familial hypophosphatemia). Taking specific forms of vitamin D, called calcitriol or dihydrotachysterol, by mouth along with phosphate supplements is effective for treating bone disorders in people with low levels of phosphate in the blood.

Underactive parathyroid (hypoparathyroidism). Taking specific forms of vitamin D, called dihydrotachysterol, calcitriol, or ergocalciferol, by mouth is effective for increasing calcium blood levels in people with low parathyroid hormone levels.

Softening of the bones (osteomalacia). Taking vitamin D3 by mouth is effective for treating this condition.

A bone disorder that occurs in people with kidney disease (renal osteodystrophy). Taking a specific form of vitamin D, called calcitriol, by mouth helps to manage low calcium levels and prevent bone loss in people with kidney failure.

Rickets. Taking vitamin D by mouth is effective for preventing and treating rickets. A specific form of vitamin D, called calcitriol, should be used in people with kidney failure.

Vitamin D deficiency. Taking vitamin D by mouth is effective for preventing and treating vitamin D deficiency.

Likely Effective for

Bone loss in people taking drugs called corticosteroids. Taking vitamin D by mouth prevents bone loss in people taking drugs called corticosteroids. Also, taking vitamin D alone or with calcium seems to improve bone density in people with existing bone loss caused by using corticosteroids.

Weak and brittle bones (osteoporosis). Taking vitamin D3 by mouth along with calcium seems to help prevent bone loss and bone breaks in people with osteoporosis.

Psoriasis. Applying vitamin D in the form of calcitriol, calcipotriene, maxacalcitol, or paricalcitol to the skin can help treat plaque-type psoriasis. Applying vitamin D along with corticosteroids seems to work better than applying vitamin D or corticosteroids alone. But taking vitamin D by mouth doesn't seem to help.

Possibly Effective for

Hay fever. Taking vitamin D by mouth seems to reduce symptoms of hay fever in adults and children. But it isn't clear if taking vitamin D during pregnancy can help to prevent hay fever in the child after birth.

Cavities. Taking vitamin D2 or D3 by mouth reduces the risk of cavities by 36% to 49% in infants, children, and adolescents.

Heart failure. Taking vitamin D by mouth can help reduce the risk of developing heart failure in some people. But it doesn't seem to help patients who already have heart failure.

Bone loss in people with overactive parathyroid (hyperparathyroidism-related bone loss). Taking vitamin D3 by mouth seems to reduce parathyroid hormone levels and bone loss in people with a condition called hyperparathyroidism.

Infection of the airways. Taking vitamin D by mouth helps prevent respiratory infections in children. But taking vitamin D by mouth during pregnancy doesn't seem to reduce the risk of these infections in the child after birth. It also doesn't help prevent infections in adults.

Preventing tooth loss (tooth retention). Taking calcium and vitamin D3 by mouth appears to prevent tooth loss in elderly people.

Possibly Ineffective for

Heart disease. Taking a vitamin D supplement by mouth doesn't seem to prevent heart disease, heart attack, stroke, or other serious heart-related events in most people.

Critical illness (trauma). Giving vitamin D to people with low vitamin D levels who are in the hospital with a critical illness doesn't make them more likely to live.

Fractures. Taking vitamin D by mouth doesn't seem to prevent fractures in older people who do NOT have osteoporosis.

High blood pressure. Taking vitamin D by mouth doesn't seem to lower blood pressure in most people with high blood pressure. But it might help people who have very low blood levels of vitamin D.

A mental disorder marked by hallucinations and delusion (psychosis). Taking vitamin D by mouth doesn't seem to improve symptoms of psychosis in adults.

Prostate cancer.  Taking vitamin D by mouth doesn't seem to reduce cancer progression or death from prostate cancer.

Tuberculosis.  Most research shows that taking vitamin D by mouth doesn't help to cure tuberculosis or make it less severe.

There is interest in using vitamin D for a number of other purposes, but there isn't enough reliable information to say whether it might be helpful.

Side Effects

When taken by mouth: Vitamin D is likely safe when taken in recommended amounts. Most people don't experience side effects with vitamin D, unless too much is taken. Some side effects of taking too much vitamin D include weakness, dry mouth, nausea, vomiting, and others. Taking vitamin D for long periods of time in doses higher than 4000 IU (100 mcg) daily is possibly unsafe and may cause very high levels of calcium in the blood.

Special Precautions and Warnings

When taken by mouth: Vitamin D is likely safe when taken in recommended amounts. Most people don't experience side effects with vitamin D, unless too much is taken. Some side effects of taking too much vitamin D include weakness, dry mouth, nausea, vomiting, and others. Allergic Rhinitis (Hay Fever) | Dr Ashwani Kansal

Allergic Rhinitis (Hay Fever)

Allergic rhinitis, also called hay fever, is an allergic reaction that causes sneezing, congestion, itchy nose and sore throat. Pollen, pet dander, mold and insects can lead to hay fever symptoms. Hay fever can make you feel awful, but you can find relief with lifestyle changes, allergy medications and immunotherapy (allergy shots)

What is allergic rhinitis (hay fever)?

Allergic rhinitis (hay fever) is an allergic reaction to tiny particles in the air called allergens. When you breathe in allergens through your nose or mouth, your body reacts by releasing natural chemical called histamine. Several indoor and outdoor allergens cause hay fever. Common causes include dust mites, mold, pet dander and pollen from trees and plants.

Symptoms of hay fever include sneezing, nasal congestion and irritation of the nose, throat, mouth and eyes. Allergic rhinitis is not the same as infectious rhinitis, otherwise known as the common cold. Hay fever is not contagious.

When do people usually get hay fever?

You can have hay fever any time of the year. Seasonal allergies occur in the spring, summer and early fall when trees and weeds bloom and pollen counts are higher. Perennial allergies can happen year-round. They result from irritants that are always around, such as pet dander, cockroaches and dust mites.

How common is allergic rhinitis (hay fever)?

Hay fever is very common. Around 15% to 20% of the population has allergic rhinitis. Millions of children and adults have hay fever every year.

Who might get allergic rhinitis?

Allergies are inherited (passed down through families). You’re more likely to have hay fever if you have a parent or family member with allergies. People who have asthma or eczema are more likely to develop hay fever.

SYMPTOMS AND CAUSES

What causes allergic rhinitis (hay fever)?

Allergic rhinitis occurs when your body’s immune system reacts to an irritant in the air. The irritants (allergens) are so tiny that you can easily inhale them through your nose or mouth.

Allergens are harmless to most people. But if you have hay fever, your immune system thinks the allergen is intruding. The immune system tries to protect your body by releasing natural chemicals into your bloodstream. The main chemical is called histamine. It causes mucous membranes in the nose, eyes and throat to become inflamed and itchy as they work to eject the allergen.

Seasonal and perennial allergies can result from many allergens, including:

Dust mites that live in carpets, drapes, bedding and furniture.

Pollen from trees, grass and weeds.

Pet dander (tiny flakes of dead skin).

Mold spores.

Cockroaches, including their saliva and waste.

Food allergies can also cause inflammation in the nose and throat. If you think you’re having an allergic reaction to something you ate, get medical help right away. Food allergies can be life-threatening.

What are the symptoms of allergic rhinitis (hay fever)?

Hay fever symptoms can appear throughout the year. Outdoor allergies are worse in the spring, summer and early fall. In warm weather, weeds and flowers bloom, and pollen counts are higher. Indoor allergies, such as those that result from pet dander and dust mites, can get worse in winter because people spend more time indoors.

Symptoms of hay fever include:

Nasal stuffiness (congestion), sneezing and runny nose.

Itchy nose, throat and eyes.

·         Headaches, sinus pain and dark circles under the eyes.

·         Increased mucus in the nose and throat.

·         Fatigue and malaise (general feeling of discomfort).

·         Sore throat from mucus dripping down the throat (postnasal drip).

Wheezing, coughing and trouble breathing.

DIAGNOSIS AND TESTS

How is hay fever diagnosed?

Your healthcare provider will examine you, ask about your symptoms and evaluate you for other conditions, such as a cold or asthma. To measure your antibodies to specific allergens, your provider may take a blood sample and send it to a lab for testing. This blood test is called an immunoglobulin E (IgE) test. It can detect all types of allergies, including food allergies.

Your provider may recommend a skin prick test to determine what allergens are causing your symptoms.

What is a skin prick test?

This common test is painless and accurate, though it may be a little uncomfortable. Your provider places a small sample of different allergens on your skin (usually on your forearm or back) and scratches or pricks the skin with a needle. Scratching the skin allows the allergen to get under the surface.

If you’re allergic to the allergen, the area will become red, itchy and irritated in 15 to 30 minutes. You may develop raised, hive-like welts called wheals that show an allergic reaction. A skin prick test is a safe, effective way to determine which allergens are causing your symptoms

How is allergic rhinitis (hay fever) treated?

Several allergy medications can improve symptoms and help you live with hay fever. These treatments come in many forms, includingliquids, pills, eye drops, nasal sprays and injections. Talk to your provider before taking any medication, especially if you’re pregnant or have other health concerns. Your provider may suggest:

Antihistamines: Antihistamine medications are available with a prescription or over the counter. They work by blocking the histamine that your body releases during an allergic response. Antihistamines come as pills, liquids, eye drops, nasal sprays and inhalers. They include:

Ebastine

Bilastine

Loratadine

Fexofenadine

Levocetirizine

Antihistamines can cause drowsiness. Avoid alcohol when taking antihistamines, especially if you’re going to drive.

Decongestants: These medications relieve congestion in the nose and sinuses. You can take decongestants by mouth (in pill or liquid form) or use a nasal spray. They include:

Nasal spray

Phenylephrine nasal spray

Otrivin or Nasivion nasal spray

Decongestants can increase blood pressure and cause headaches, trouble sleeping and irritability. Nasal decongestants can be addictive if used longer than five days. Be sure to talk to your provider before taking them.

Corticosteroid nasal sprays: These sprays and inhalers reduce inflammation and relieve symptoms of hay fever. The most common nasal sprays are Free Air AZ, Ryaltris AZ, Azeflo FT, Duonase,Flonase®, Nasacort® and Rhinocort®. Side effects include headaches, nasal irritation, nosebleeds and cough.

Leukotriene inhibitors: During an allergic reaction, the body releases leukotriene, histamines and other chemicals that cause inflammation and hay fever symptoms. Available only with a prescription, these pills block leukotriene. The most common leukotriene inhibitor is montelukast . Some people experience changes in mood, vivid dreams, involuntary muscle movements and skin rash when taking this medication.

Immunotherapy: This treatment works by helping your body learn to tolerate allergens. Your provider gives you a series of injections (allergy shots) with a small amount of the allergen. Every time you get a shot, your provider increases the amount of the allergen. Over time, your immune system develops immunity to the allergen and stops launching a reaction to it.

Your provider might recommend immunotherapy in the form of a pill that you place under your tongue.

PREVENTION

How to prevent hay fever?

There is no way to prevent hay fever, but lifestyle changes can help you live with allergies. You can relieve hay fever symptoms by avoiding irritants as much as possible. To reduce symptoms, you should:

Avoid touching your face and rubbing your eyes or nose.

Close windows in your home and car during the spring, summer and early fall when pollen counts are higher.

Enclose pillows, mattresses and box springs in dust mite covers.

Keep pets off couches and beds, and close doors to bedrooms you don’t want them to enter.

Use filters in your vacuum cleaner and air conditioner to reduce the amount of allergens in the air.

Wash your hands often, especially after playing with pets.

Wear a hat and sunglasses to protect your eyes from pollen when you’re outside. Change your clothes as soon as you come indoors

YOG NIDRA | Yoga Nidra Is Like Meditation | Dr Ashwani Kansal

YOG NIDRA

Style of yoga is that a 45-minute session could leave you feeling like you indulged in a peaceful three-hour nap.

If you’re ready for an easy, pose-free way to slow down and recover from the stressors in your life, read on to discover how yoga nidra could be the answer.

How yoga nidra differs from meditation

Yoga nidra involves slowing down and chilling out. So does meditation. While some people tend to lump them together, they really are two different practices.

“Yoga Nidra Is Like Meditation

“There are overlaps, but there also are key differences. With yoga nidra, you are lying down and the goal is to move into a deep state of conscious awareness sleep, which is a deeper state of relaxation with awareness. This state involves moving from consciousness while awake to dreaming and then to not-dreaming while remaining awake — going past the unconscious to the conscious.” Bar says that this practice is guided like some meditation practices, but it’s very structured.

“With meditation, you’re sitting and in a waking state of consciousness while focusing the mind and allowing thoughts to come and go. Meditation makes it possible for us to get to the theta state — the state we go through to get to the delta state, which is the place of the deepest sleep cycle. The delta state is a deep healing state. That’s where we’re trying to get through yoga nidra. In this state, the body and mind rest and the consciousness is awake.”

Benefits of Yoga Nidra

Yoga nidra works with the autonomic nervous system. The autonomic nervous system regulates processes of the body that take place without a conscious effort (heartbeat, breathing, digestion and blood flow). This system also includes the sympathetic and parasympathetic nervous systems.

Meditation helps us calm the sympathetic nervous system; mainly, our fight-or-flight response. “We do a meditation practice to basically calm the sympathetic, or fight-or-flight and activate the parasympathetic more. There’s such a benefit when those are balanced overall for immunity, digestion and stress management. But in this deeper relaxation, the pineal gland is activated and that releases the hormone melatonin.”

Melatonin is a powerful antioxidant. It can also help manage immune function, blood pressure, cortisol levels and induce restful sleep.

Meditation and yoga nidra were both effective in reducing anxiety and stress, yoga nidra seemed to be more effective in reducing anxiety.

Yoga nidra can be a useful tool in reducing both cognitive and physiological symptoms of anxiety.

10 Steps of A Yoga Nidra Practice

You can do it at home with the help of YouTube or a meditation app. You don’t need fancy equipment either. You can lie flat on your back on a yoga mat or a blanket with a bolster or pillow supporting your lower back, spine and your head. You can even put a blanket or pillow under your knees.

There are 10 stages of a yoga nidra practice.

 “10 Stages of Yoga Nidra.”

1. Connect to your heart’s deepest desire. Focus on a lifelong goal or something that relates to your health.

2. Visualize reaching this goal and feel the joy that comes with accomplishing it.

3. Set an intention. Think about why you’re practicing — to get centered, to put some self-care on the schedule — whatever the reason, keep it at the forefront of the yoga nidra practice.

4. Find your inner resource. This involves tapping into a safe space within the body so you can feel secure and at ease while you practice.

5. Scan your body. During a body scan, you’ll be asked to focus on certain parts or sensations throughout the body. The goal of this is to help reduce tension so you can relax.

6. Become aware of your breath. Pay attention to how air is flowing in and out of your body. Take note of how it comes in your nostrils and how your abdomen rises and falls. This can help you slow down and breathe evenly.

7. Welcome your feelings. If you had a rough day, embrace it. You don’t have to ignore the tough stuff, but in recognizing it, you can also think about the opposite of whatever emotion you’re feeling to balance things out.

8. Witness your thoughts. Similar to step six, you want to observe your thoughts in the moment without judging or trying to block them out. Should any negative thoughts about yourself surface, think about the positive side of things to ease tension.

9. Experience joy. If you start to feel blissed out, embrace it and let it wrap around your body.

10. Observe your “self.” Be aware of your personality and how you might be feeling. In other words, your sense of “I-ness.” Then, consider yourself an observing witness. This will help you wake up more aware and in tune with your feelings.

Reflect on your practice. When you finish, think about how you feel and what you were able to tap into during your session. Then, think about how you can bring the peace or joy you might be feeling into daily life whether times are good or bad. Don’t rush out of your practice. Take a few minutes to transition back into the waking state of life.
